Dear readers, I am general practitioner living in Spain. French medical formation, I am interested in alternative medicine and cupping. This blog is for presenting two books on cupping facial, but also doing a physiology supplement on cupping and alternative therapies. For the moment, two books on Amazon: LYMPHATIC FACIAL CUPPING : it's for beginners or for trainers. It is a practical tool for their basic courses. INTEGRATIVE FACIAL CUPPING: To go further, to deepen the knowledge. Lymphatic drainage + facelift + puffiness eyes, face. I will be happy to hear your opinion on the books (print quality, new ideas). The good and the bad.

THE DOUBLE FACE OF THE PERIORBITAL ZONE: the ophtalmic and mandibular branches (french - english version)

THE DOUBLE FACE OF THE PERIORBITAL ZONE

 Looking for the trigeminal branches...

By passing the suction pad or hands during a massage in the periorbital area, we stimulate the sensory roots of the trigeminal nerve.

Indeed, it has three branches (ophthalmic, maxillary and mandibular).

The edge of the orbit, precise anatomical zone, is under the influence of the trigeminal, but by its two branches (diagram A).

  • On the upper edge, it is the ophthalmic branch.
  • On the lower edge is the maxillary branch.

Explanation of the path of the surface anatomy

The upper edge of the orbit

It receives the ophthalmic branches of the trigeminal nerve:

·       the lachrymal nerve for the lateral part of the upper eyelid.

·       the frontal nerve for the medial part of the medial eyelid (supraorbital and supratrochlear nerve).

The lower edge of the orbit

It receives a maxillar branch of the trigeminal:

- A terminal branch of the infraorbital nerve exits the infraorbital foramen) divides into three branches:

- the nasal nerve (for the wing of the nose),

- the superior labial nerve,

And for what concerns us the lower palpebral nerve.

Diagram A

Practical consequences

Anatomical identification of supraorbital and infraorbital nerves

The emergence of the supraorbital nerve will be quite easy, drawing a line through the middle of the pupil and the corner of the mouth.
The emergence of the infraorbital nerve will be quite easy drawing a line through the middle of the pupil and the corner of the mouth. About 2-3 cm below the lower orbital rim and 2 cm from the nose.

Which allows to place suction cups at these levels (diagram B)

CUPPING AND OPHTHALMIC NERVE IN SURFACE ANATOMY (english - french version)

CUPPING AND OPHTHALMIC NERVE IN SURFACE ANATOMY

ENGLISH VERSION

TRIGEMINAL NERVE AND ITS OPHTHALMIC BRANCHES IN SURFACE ANATOMY

When placing a suction cup or a hand on the face or neck: it is the trigeminal nerve that is stimulated. This is the essential nerve of the sensitivity of the face and neck.

Before projecting to the surface, like ramifications of the branches of a tree, the trigeminal nerve divides into three branches from a trigeminal ganglion called Gasser. These three branches are  ophtalmic branch (V1),  maxillary branche (V2 ) and mandibular branch ( V3).

Then, as we said in a previous article, peripheral nerve roots come out through holes in the bones of the face. These roots will "scatter" over the entire surface of the skin, they have a skin receptor field.

What is interesting: it is to know the number of these emergent branches on the surface:
- For the ophthalmic branch of the trigeminal we have 5 branches.
- For the maxillary branch of the trigeminal we have 3 branches.
- For the mandibular branch of the trigeminal we have 3 branches.

Here is a diagram tracing the emergence of these branches (diagram A).
DIAGRAM A

ADDITIONAL NOTE

  • The wings of the nose are in the area of sensory innervation of the maxillary nerve! (see diagram additional note).

ADDITIONAL NOTE :Nose wings and nasal sidewall difference.

DIAGRAM A bis (the surface and endocranial anatomy).

CONSEQUENCES OF THIS SURFACE ANATOMY

It optimizes the placement of suction cups (without using acupuncture points, which we do not neglect). Indeed, we rely on anatomical bases. The mechanical stimulation by the suction cups will stimulate the fibers sensitive to the tact of this nerve (Type A Beta fibers). This in turn stimulates the activity of inhibitory interneurons of nonspecific nociceptive neurons by parasiting the nerve signal at the origin of the nociception.

The example of the supraorbital nerve

The emergence of the supraorbital nerve will be quite easy, drawing a line through the middle of the pupil and the corner of the mouth. (Diagram B).

Cupping and interconnection of cranial nerves (english and french version)

Cupping and interconnection of cranial nerves

ENGLISH VERSION

A SENSITIVO-MOTRICITY DISTINCTION

Physiologically, the nerves of the face that are essentially the facial nerve (7th pair) and the trigeminal nerve (5th pair).
  • The trigeminal nerve is the nerve of the sensibility of the face.
  • The facial nerve is the nerve of the motriticity of the face.

Of course, this distinction between motricity and sensitivity function is not 100%. The trigeminal nerve has a mandibular root with motor and sensory rules.

The trigeminal nerve, sensitive nerve of the face, neck, meninges.

ANATOMICAL PROXIMITY AND ANATOMICAL ANASTOMOSIS

Because of their anatomical proximity and their functions: these two nerves have anastomosis in the different territories of the face, ie in surface area.
But also, these two nerves are connected in deep territories at the level of the medulla oblongata.
Indeed, the nerve VII needs a sensory feedback to coordinate the contraction of these muscles.

Interestingly, at the level of the medulla oblongata is the spinal nucleus of the trigeminal nerve. The latter receives afferences from different cranial nerves (the IX, X) but also the VII (= facial nerve).

The action of cupping in these interconnections.

In the cupping, we will try to stimulate the different anatosmoses, the different emergences of the nerves at the level of the face.
The stretching of the skin by a suction cup will mechanically stimulate the large fibers with tactile sensitivity (type A) and will parasitize the painful information on the spinal nucleus of the trigeminal nerve.

ENGLISH  VERSION

Remarks:

Beyond the side of classical medicine and anatomy.

The cupping therapist must take into account the numerous emergences of the cranial nerves at the level of the face.

Placement of the suction cups:

Taking into account the surface branches of the cranial nerves will allow a good placement of the suction cups.

Therefore, know and review the anatomy of the trigeminal nerve for facial cupping.

All these nerve branches on the surface are possible places of placement of suction cups.

CUPPING ACTS ON LOCAL BLOOD FLOW AND LOCAL ANATOMICAL STRUCTURE

CUPPING ACTS ON LOCAL BLOOD FLOW AND LOCAL ANATOMICAL STRUCTURE

 Transvascular flow's angel !

Interstitial and intravascular flows and their corresponding microenvironments. A blood vessel is shown, with insets showing the transvascular and glycocalyx regions.
By definition, interstitial flow refers to fluid flow around an interstitial cell: a cell attached to extracellular matrix in three dimensions. For endothelial cells, flow within vessels occurs only on the apical surface. Where there is a glycocalyx, flow might percolate through that network of proteoglycans and cause complex stresses on the cell surface, but fluid flow is still two-dimensional with respect to the cell surface. However, the intimate cell–matrix–flow interactions at the glycocalyx–cell interface might lead to effects similar to those in three dimensions. Intravascular pressure can drive flow through the vascular wall, but endothelial cells and smooth muscle cells will experience flow stresses through cell–cell connections and
through cell–matrix connections as in true interstitial flow.
