SURFACE ANATOMY AND CUPPING : the case of mandibular nerve (english - french version).

CUPPING AND MANDIBULAR NERVE IN SURFACE ANATOMY

ENGLISH VERSION

TRIGEMINAL NERVE AND ITS MANDIBULAR BRANCHES IN SURFACE ANATOMY

When placing a suction cup or a hand on the face or neck: it is the trigeminal nerve that is stimulated. This is the essential nerve of the sensitivity of the face and neck.

Before projecting to the surface, like ramifications of the branches of a tree, the trigeminal nerve divides into three branches from a trigeminal ganglion called Gasser. These three branches are  ophtalmic branch (V1),  maxillary branche (V2 ) and mandibular branch ( V3).

Then, as we said in a previous article, peripheral nerve roots come out through holes in the bones of the face. These roots will "scatter" over the entire surface of the skin, they have a skin receptor field.

What is interesting: it is to know the number of these emergent branches on the surface:
- For the ophthalmic branch of the trigeminal we have 5 branches.
- For the maxillary branch of the trigeminal we have 3 branches.
- For the mandibular branch of the trigeminal we have 3 branches.

Here is a diagram tracing the emergence of mandibular branches (diagram A).

Diagram A

ADDITIONAL NOTE

  • This nerve has endings on the surface, but also like many nerves: it also has internal branches, that is, towards the inside of the tissues. They do not emerge on the surface of the skin (in yellow on diagram B).
Diagram B

  • The nerve branches drawn in the diagram, do not represent the reality of their spreading (see "their bursting" in several ramifications). We will illustrate it in a next paragraph with the mental nerve.

CONSEQUENCES OF THIS SURFACE ANATOMY

It optimizes the placement of suction cups (without using acupuncture points, which we do not neglect). Indeed, we rely on anatomical bases. The mechanical stimulation by the suction cups will stimulate the fibers sensitive to the tact of this nerve (Type A Beta fibers). This in turn stimulates the activity of inhibitory interneurons of nonspecific nociceptive neurons by parasiting the nerve signal at the origin of the nociception.

The example of the mental nerve

The mental nerve does not have a simple ending. It "divides" into many small branches. It is obliged to innervate the labial mucosa and the gengiva to fulfill his role.

The following diagram shows this "explosion" of small branches.
Here we have the mental nerve, but we could have taken an example with other nerves on the surface.

Diagram C
Diagram D
Classically, the location of the mental foramen and its nerve is in the extension of the vertical passing through the supraorbital foramen. Some speak 2.8 cm from the center line (diagram D).
Certainly, we can place a sucker or pressotherapy (acupressure) at this level, but we can massage the whole area of the chin, because of the dispersal ramifications. This will directly stimulate the sensory tactile fibers of the trigeminal nerve (Beta fibers).

THE DOUBLE FACE OF THE PERIORBITAL ZONE: the ophtalmic and mandibular branches (french - english version)

THE DOUBLE FACE OF THE PERIORBITAL ZONE

 Looking for the trigeminal branches...

By passing the suction pad or hands during a massage in the periorbital area, we stimulate the sensory roots of the trigeminal nerve.

Indeed, it has three branches (ophthalmic, maxillary and mandibular).

The edge of the orbit, precise anatomical zone, is under the influence of the trigeminal, but by its two branches (diagram A).

  • On the upper edge, it is the ophthalmic branch.
  • On the lower edge is the maxillary branch.

Explanation of the path of the surface anatomy

The upper edge of the orbit

It receives the ophthalmic branches of the trigeminal nerve:

·       the lachrymal nerve for the lateral part of the upper eyelid.

·       the frontal nerve for the medial part of the medial eyelid (supraorbital and supratrochlear nerve).

The lower edge of the orbit

It receives a maxillar branch of the trigeminal:

- A terminal branch of the infraorbital nerve exits the infraorbital foramen) divides into three branches:

- the nasal nerve (for the wing of the nose),

- the superior labial nerve,

And for what concerns us the lower palpebral nerve.

Diagram A

Practical consequences

Anatomical identification of supraorbital and infraorbital nerves

The emergence of the supraorbital nerve will be quite easy, drawing a line through the middle of the pupil and the corner of the mouth.
The emergence of the infraorbital nerve will be quite easy drawing a line through the middle of the pupil and the corner of the mouth. About 2-3 cm below the lower orbital rim and 2 cm from the nose.

Which allows to place suction cups at these levels (diagram B)
